Who is eligible
- Must have diabetes
- Must be a high school student or currently enrolled in college
- Must have excelled in academics and tennis
- Must have strong leadership and community involvement
What you'll need
- Essay (2,000 characters max): "Please tell us how tennis and diabetes have impacted your life."
- Most recent unofficial or official high school or college transcript displaying name and cumulative GPA
- At least one letter of recommendation from a teacher, counselor, coach, or mentor
- Optional second letter of recommendation
- Optional Resume
- Details on tennis connection, rankings, and coach/program contact details
- Financial information (Adjusted Gross Income and household size)
- Financial barriers statement (350 words max)
How to apply
- Fill out the online application form via the USTA Foundation portal.

- Input details about your tennis experience, rankings, and coach's contact information.
- Provide financial information including household size and AGI, and complete the financial barriers statement.
