Who is eligible

  • Must document his/her (or the parent's) sickle cell disease diagnosis
  • Must reside in the State of New Jersey
  • Must attend, be accepted, or have applied to an accredited university, college, or technical/vocational school
  • Must have a GPA of 2.5 or greater on a 4.0 scale

What you'll need

  • Sickle cell disease confirmation form
  • Most recent official transcript
  • Letter of acceptance from the prospective post-secondary school
  • Personal written essay (not to exceed 1,000 words): "include how you face your challenges, your educational plans, and your career objectives"
  • Two letters of recommendation from non-family members (one must be from a school official, e.g. teacher or guidance counselor)
  • Proof of residency (utility bill, driver’s license, or other)
  • Clean and appropriate current picture
